I/We, the parents and or guardian of the above named candidate for a position on a league team, desire to become a "Member" of the Brookwood Athletic Association, Inc., and hereby give my/our approval to participate in any and all league activities. I/We assume all risk and hazards incidental to such participation, including transportation to and from the activities; and I/We do hereby waive, release, absolve, indemnify and agree to hold harmless the local league, the chartering organization, the organizers, sponsors, participants and persons transporting my/our child to and from activities; for any claim arising out of an injury to my/our child, whether the result of negligence or from any other cause, except to the extent and in the amount covered by accident and liability insurance. I/We understand that the insurance carried by this league covers only the amount that is not paid by my/our carrier. I/We will furnish a certified birth certificate of the above named candidate to league officials upon request. I/We hereby give permission to all BAAB officers, managers and coaches to authorize emergency care for the named candidate.

Member agrees that he or she will at all times hereafter indemnify the League, its officers and directors, managers, coaches, other members, players, officials, and other individuals assisting in the League, against any losses, injuries or damage to Member's child, and hold harmless BAAB and release BAAB from any and all claims, actions, and/or demands that could be claimed as a result of said child's participation in BAAB activities, including transportation to and from the activities.

MEMBER CODE OF CONDUCT: I further agree to abide by the League's Bylaws and the Baseball and Softball Rules adopted by the Board of Directors Members and an adult will at all times conduct themselves as Ladies and Gentlemen and exemplify good sportsmanship and character. Un-sportsmanship like behavior and violation of BAAB rules may result in temporary or permanent suspension against the offending individual(s).

MEMBER'S RESPONSIBILITIES: I further agree and understand that BAAB is a volunteer organization and that my/our child's participation is conditioned upon my/us volunteering at least two (2) hours to work at the concession stand, or where designated by BAAB.  I also understand that each Member(s) is/are responsible for assisting their child's league team with field maintenance and clean up and whatever may be required from time to time by your child's league team. I also agree to return any equipment issued to my child by the League.

EMERGENCY MEDICAL AND SURGICAL TREATMENT: I hereby give permission for the League (in my absence) to obtain medical services for my child in case of a medical emergency or injury. BAAB will not be responsible for injury to any player, Member, coach, manager, spectator or any other participant of any League game or other function.

GUIDELINES FOR PARENTS:

• Set a good example for the children. Yelling at umpires shows great disrespect for the game of baseball and sets a poor standard of sportsmanship. It is the manager's job alone to discuss any problems with the umpires.
• Encourage your kids AND the rest of the kids on the team. Let's pump all these kids up so they'll have the times of their lives out there. Keep the comments positive and the results will be too!
• Bring problems to the attention of the manager IN PRIVATE. Arguing with the manager in public is detrimental to getting a successful resolution.
• Notify the managers of any absences as soon as you become aware of them. Substitution and playing time rules are complex, so managers need time to put together the plan. Last minute changes can be very difficult to accomplish.
• Please be on time to all events. Try to arrange for alternate transportation if the kids can't get there on time. Not leaving enough time to warm up is a good way to get a sore arm.
• Please pick your child up promptly. Managers wait until all players are accounted for so try not to add more to the time commitment they already have.

I agree that the applicant will play on the team to which he or she is assigned and will attend substantially all practices and league games. I am aware that noncompliance may result in my child's removal from the team. I have read and understood the General Information supplied with this Application and on the league's website.

I agree to abide by the BAAB Parental Code of Conduct as follows:

• I will not engage in arguments, use foul or abusive language, or exhibit any acts of verbal or physical aggression.
• I will exhibit good sportsmanship to make this an enjoyable experience for all participants.
• I will remember that I am spectator of youth sports and not belittle players of the home or visiting teams.
• I will not perform actions to distract the batter, pitcher or other players while the game is being played.
• I will place the physical and emotional well being of all players ahead of the desire to win.
• I will treat all managers, coaches, and umpires with dignity and respect.
• I will encourage my daughter/son to treat other players, coaches, fans, and officers with respect.
• I will adhere to requirements to maintain an environment free of drugs,  alcohol, and tobacco use.
